In order to make your first visit as easy as we can, please print off these forms and fill them out prior to your first appointment. Please print neatly in pen so we can quickly get your information transferred into the computer.
There are a few ways you can get them to us (#5 is our favorite:
1. Just bring them completed to your appointment
2. Fill them out at home and fax them in to us at 281.579.1808

4. For the computer savy: fill them out by hand at home, scan them in, and send them as a PDF to ktdentist@aol.com
5. This next one is our favorite! It saves paper, we are guaranteed to be able to read it and honestly, it is just kind of cool!
So here we go, for the really, really savy: Open the PDF document (the new patient forms). Save the document to a file, I would just save it to the desktop to make it easy. Then click TOOLS -TYPEWRITER - SHOW TYPEWRITER. Click the typewriter icon and place the cursor over where you want to type! Please keep in mind you have to keep clicking the typewriter tool for each section, but the easy part is you can just move the typewriter tool bar all over your screen to make it easier! Complete the documents, save them to your desktop and email the file to ktdentist@aol.com. We will shoot you an email back confirming that we have received it!
